What to bring and what to expect.
A first consultation begins with the patient’s story, symptoms, timeline, prior care, daily function, imaging, and the questions that matter most. The visit is designed to connect those pieces before a recommendation is made.
Arrange access to the actual MRI, CT, X-ray, or other relevant imaging before the visit whenever possible. Actual imaging, not only the written report, is required for surgical review. Prior clinic and operative notes, a medication list, treatment history, insurance information, and a written question list can also be useful.

Request your consultation
Use the appointment request form or call 754-300-0817. The team must confirm your appointment; submitting the form does not reserve a visit time.
Confirm the visit details
Ask the team to confirm your appointment location, insurance participation, any referral or authorization requirements, and how to complete new-patient forms.
Arrange your records
Gather your imaging files and written reports, prior consultation or operative notes, and current medication list. Ask the team how to send records securely before the visit. Do not upload medical records or enter medical details in the general contact form.

Where do I complete new-patient forms?
Call the practice for current intake instructions and a secure way to complete your forms.
What if I do not have the images yet?
Tell the team when requesting your visit. They can explain which records to request and how to arrange secure delivery.
Can I request communication or accessibility support?
Tell the team about communication, mobility, or other accessibility needs when booking so you can discuss arrangements before your visit.
